Incident Investigation

This online course is designed to familiarize aquatic professionals with the common procedures of conducting an incident investigation, specifically an incident involving an injury. The crux of why incident investigation is critical is to implement corrective actions to prevent further incidents. There is currently not an OSHA regulatory driver for incident investigation. Section 5a1 of the Occupational Safety and Health Act of 1970 states, “Each employer shall furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees.” If there is a serious injury, OSHA has the right to investigate and is obligated to do so in the case of a workplace fatality. A proper investigation also protects the employer’s liability to lawsuits and unwarranted work-comp claims.

Incident Investigation training focuses on the following objectives, while the online format allows students to learn at their own pace:

  • Recognize the purpose and benefits of an incident investigation
  • Identify common practices to follow and information to be gathered during an investigation
  • Describe documenting and reporting responsibilities
  • Recognize the importance of and common practices for determining root cause
  • Discuss the importance of implementing corrective actions
